BJC HealthCare System is hiring a Full-Time Case Management Assistant for St. Peters Hospital (10 Hospital Dr., St Peters, MO 63376)

 Position Details:

  • Day Shift Hours: 8am-4:30pm (Mon-Fri)
  • Supporting administrative activities of required CMS letters to patients, Assisting with authorization activities, Obtaining post discharge/transition of care appointments, management of incoming messages, faxes and supporting the team of Case mangers and social workers with resources, referrals and faxing, etc.

Experience:

  • General healthcare experience
  • Authorization, EPIC, and office experience (preferred)
  • Microsoft office experience

  • Medical Office Secretary

Requirements:

Overview

Barnes-Jewish St. Peters Hospital proudly serves the health care needs of families in St. Charles, Lincoln and Warren counties. The 102-bed community hospital provides comprehensive inpatient and outpatient services including the Siteman Cancer Center at Barnes-Jewish St. Peters Hospital, the Breast Health and Women’s Center, the Cardiology Center, the Neurosurgery Center and the Outpatient Surgery and Endoscopy Center.

Barnes-Jewish St. Peters Hospital is an Accredited Chest Pain Center and carries a Level II stroke certification. Additionally, the hospital’s medical office building houses offices for specialists in primary care, cardiology, obstetrics, dermatology, surgery and pulmonology. Barnes-Jewish St. Peters Hospital also offers outpatient services including physical therapy, a sleep lab, and cardio-pulmonary rehabilitation.

At Barnes-Jewish St. Peters Hospital, every decision is made in the best interest of patients and families. The Patient Care Team is responsible for accesses, plans, implements, and evaluates each patient's plan of care to facilitate a clinically safe transition home.


Preferred Qualifications

Role Purpose

A Case Management (CM) Assistant functions under the direction of CM services clinical personnel, assisting with activities not requiring clinical expertise. CM Assistants possess excellent communication, organizational and computer skills.

Responsibilities

  • Supports Case Managers and Social Workers as assigned to secure post-acute providers of care for patient post-discharge needs by collecting, entering, and sending referrals, then monitoring for follow-up responses until referral process is complete.
  • Supports CM to provide the Centers for Medicare & Medicaid Services communications and forms to patients in a safe, timely, and effective manner, and facilitates appropriate follow up to enhance the quality of the services to patients.
  • Facilitates multiple care aspects under the direction of the care coordinator for discharge planning such as: assist with referrals and/or appointments to Physicians and/or clinics or coordinates transportation from hospital upon discharge as needed.
  • .Performs a variety of routine clerical activities in support of the Case Management department.
